Our Partial Hospitalization Program (PHP) is committed to protecting the privacy of your health information. We are required by law to:

- Notify you following a breach of your unsecured protected health information.
How We May Use and Disclose Your Health Information:
For Treatment
We may use and disclose your health information to provide you with treatment and services. For example, we may share your information with doctors, nurses, case managers, and other healthcare professionals involved in your care to coordinate your treatment plan.
For Payment
- We may use and disclose your health information to bill and receive service payments. This may include sending information to your insurance company or other responsible parties5.
- For Healthcare Operations
- We may use and disclose your health information for our day-to-day operations, such as quality assessment, improvement activities, and evaluation of our staff performances.
Your Rights Regarding Your Health Information:
You have the following rights regarding your health information:
- Right to inspect and copy your health record
- Right to request amendments to your health record
- Right to receive an accounting of disclosures of your health information
- Right to request restrictions on specific uses and disclosures
- Right to receive confidential communications
- Right to receive a paper copy of this Notice
Changes to This Notice
We reserve the right to change this Notice and apply the new Notice to health information we already have and any information we receive in the future. We will post a copy of the current Notice in our facility and on our website.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our facility or the Department of Health and Human Services Secretary. Filing a complaint will not affect your treatment or services.
